Philippines HR Employee Approval History
Preserve HR approval events so reviewers can distinguish a draft, a request, and an authorized decision.
An approval history records the request reference, decision sought, packet version, submitted date, approver, decision time, outcome, and conditions. A completed form or a message asking for approval is not evidence that approval occurred.
A coordinator can assemble the packet, verify that the approver is identified, and link supporting records. HR or management decides the request, conditions, effective date, and any communication that follows.
Never overwrite a prior decision when a request is reconsidered. Append the new decision with its own source and timestamp so the record explains the sequence instead of displaying only the latest label.
