Philippines HR Employee Manager Feedback Log
Record manager feedback with dates and follow-up owners without turning an informal note into a formal rating.
A feedback log identifies the worker reference, work example, date, manager, requested follow-up, review point, and state. A short comment can be useful context, but it should not be presented as a rating or disciplinary finding unless the owner records it that way.
An administrator can format examples, track agreed follow-up, and remind the owner about a review point. The manager and HR decide interpretation, formal status, response, and employment action.
Keep the employee response and manager note connected but distinct. Use concrete work references and avoid filling gaps with assumptions about intent or character.
