Philippines HR Employee Name-Change Record
Route name changes with source evidence and effective dates while protecting the prior record and owner decision.
A name-change record includes the worker reference, current value reference, requested value, evidence source, received date, effective date, affected systems, and owner. A preferred display name and a legal-record correction may follow different processes.
A records coordinator can check the request for completeness, identify systems that may need review, and flag a conflict. HR or the data owner decides the required evidence, access, effective treatment, and communication.
